Dr. Shawn Hamilton began her career as a primary care physician, building and managing a panel of 2,000 patients over nearly three decades in Southern California. As a board-certified internist, she developed deep expertise in chronic disease management, preventive medicine, and the kind of whole-person care that traditional healthcare rarely has time to deliver.

Her clinical career evolved into executive healthcare leadership at two of California's most prominent institutions.

As Medical Director at AltaMed Health Services — the nation's largest non-profit Federally Qualified Health Center, serving over 700,000 patients annually across Los Angeles and Orange Counties — Dr. Hamilton oversaw clinical quality, physician performance, care coordination, and population health strategy for one of the most complex and diverse patient communities in the country. Leading medicine at scale in an FQHC demands mastery of chronic disease, health equity, cultural competency, and system-wide clinical excellence.

As Regional Medical Director at Health Net — one of California's largest health plans, serving more than three million members statewide — Dr. Hamilton operated at the intersection of clinical medicine and health policy. Her responsibilities encompassed utilization management, quality improvement, care gap closure, and ensuring that evidence-based medicine translated into real outcomes for a vast and diverse membership.

In a testament to her entrepreneurial vision and deep commitment to underserved communities, Dr. Hamilton founded and led Sowing Seeds Health — a California nonprofit organization that grew from a music ministry into a $5 million revenue healthcare company. Under her leadership as CEO and President, Sowing Seeds Health became a licensed provider of Enhanced Care Management and Community Supports services under California's landmark CalAIM initiative — connecting individuals experiencing homelessness, serious mental illness, chronic disease, and complex social barriers to the clinical and community resources they need to stabilize and thrive.

These roles required more than clinical knowledge. They demanded the ability to see a patient not just as a body in a chair, but as a whole person navigating the full complexity of life — work, family, culture, language, chronic illness, and the daily choices that either build health or erode it.
